1. Authority
I confirm that I am the player’s parent or legal guardian and have authority to authorize the application, coaching communication, participation, and payment.
2. Age-specific control
For a player age 10–12, I will submit and control the full application, payment, OnForm access, video sharing, and all communication. For any player under 18, I will remain included in every coaching communication.
3. Information and privacy
I consent to the collection and use of the minor’s limited information as described in the Privacy Policy. I understand that a waiver does not replace privacy notice or parental consent, and that I may request access, correction, deletion, or withdrawal of consent by contacting the business. Withdrawal may end the service when the information is necessary to coach safely.
4. Video and communications
I authorize private coaching use of footage that I provide through the approved process. This does not authorize public posting, testimonials, advertising, or social media use. A separate optional Media Release is required for any public use.
5. Participation
I have reviewed the Coaching and Participation Waiver, will provide safe supervision and training conditions, and will involve a healthcare professional for medical decisions.
6. Records and acceptance
The application records the consent date, applicant contact, player age group, policy versions, and technical audit data. A final attorney-approved signature process may be required before coaching begins.
